Instructions:  After clicking the link above, you will be taken to a screen that looks like this.....

Where you see the block labeled "Domain\user name", type in "OTC" followed immediately by a backwards slash "\" - the backwards slash should be immediately followed by your user name.  Then tab to the password block and insert your password.
